According to a report from the U.S. Department of Health & Human Services, March 2012, up to half of all patients from psychiatric hospitals require readmission within 1 year of discharge. Many of these readmissions can be attributed to treatment factors including noncompliance with medication and lack of outpatient and community support.

To address this vulnerability within this population, the following factors were found helpful in reducing need for psychiatric hospitalizations:

  • Medication Education

  • Facilitating follow up appointments

  • Outpatient case management
