Life Skills PRP is a unique program, offering rehabilitation services in both a day program and community based/outreach setting, that creates a supportive comfortable place for recovery and wellness, serves as a liaison and advocate for individuals in need. Our onsite day programs offer clients a safe, supportive and structured environment. Clients come during the day to participate in groups, activities, community outings and socialize with peers. Our skill-based groups and outings promote socialization, links to community resources, independent living skills, health and wellness, as well as recovery and resiliency.

Outreach Services


Our outreach services offer clients an opportunity to continue living successfully in the community while receiving individual support and case management from their rehabilitation coordinator. Our teams support clients to have better appointment management, and assist them in maintaining their independence. Outreach services may take place in the home, or in the community. Service areas include Baltimore County, Baltimore City, Montgomery County, and Prince Georges County.

Outreach Services with Shared Living


Our Baltimore location offers a unique opportunity for individuals who qualify for PRP services, to receive outreach case management in a shared living environment. All services are community based where the rehabilitation coordinator visits clients at the home. Our goal is to maintain clients independent in the community while offering support in the home. Clients sub-lease rooms where Centers is the lease-holder.
